CANDLE BAR PARTICIPATION WAIVER & RELEASE OF LIABILITY

II understand that participation in candle-making activities at ETNICO LLC involves inherent risks, including but not limited to exposure to hot wax, fragrance oils, open flame, heated equipment, and tools that may cause burns, allergic reactions, irritation, or other injuries.

I voluntarily choose to participate and assume all risks, whether known or unknown, associated with this activity. I acknowledge that improper use of equipment or failure to follow safety instructions may increase the risk of injury.

I hereby waive, release, and discharge ETNICO LLC, their owners, members, officers, employees, contractors, volunteers, and agents from any and all claims, liabilities, demands, or causes of action arising out of or related to my participation, including claims arising from ordinary negligence, to the fullest extent permitted by New Jersey law.

I acknowledge that I am responsible for informing staff of any allergies, sensitivities, medical conditions, or concerns prior to participation. I understand that products and experiences offered are not intended to diagnose, treat, cure, or prevent any medical condition.

I agree to follow all safety instructions provided during the experience and understand that failure to do so may result in removal from the activity without refund.

I grant permission for photographs and video recordings taken during the event to be used for marketing and promotional purposes.

If I am signing on behalf of a minor, I certify that I am the legal parent or guardian and consent to their participation.

I acknowledge that I have read and understand this Waiver and Release of Liability and sign it voluntarily.

We have limited space in our store, therefore we allow ONE person per candle.
